Blueprint Manager Request Form

This form is intended for requesting administrative access as a Blueprint Manager in Canvas. As per the Canvas Access Guidelines, administrative access to Canvas requires approval from a Dean or from the Office of the Provost. Upon submission, this request will be automatically forwarded for approval to the designated Dean or Office of the Provost.
